Family Session - Model Needed

This form is for a family session on Monday. June 15th from 6:30-6:45pm at a pre-selected location.

REQUIREMENTS
*Family with 3-4 people total (including parents)
*Children must be between 3-8 years of age
*Children must like being photographed
*You must be on time
*You must submit a family photo, does not need to be professional
*You must be choose your clothing for the session based on one of the color palettes below
